Condition
Crowded teeth
Crowding is a mismatch between the space available in the arch and the space the teeth need. Every treatment for it works by changing one side of that equation.

Why crowding happens
- The arches are narrow or short relative to the size of the teeth.
- The teeth are large relative to the jaws — tooth size and jaw size are inherited separately.
- Early loss of baby teeth allows the back teeth to drift forward, using up space intended for the permanent teeth.
- Late lower incisor crowding, which appears in the late teens and twenties and affects most people to some degree, including those who never had orthodontic treatment.
The three ways to create space

| Method | How it works | Considerations |
|---|---|---|
| Expansion | Widens the arch, skeletally or dentally | Appropriate where there is a genuine transverse deficiency; dental expansion beyond the bone is unstable |
| Interproximal reduction | Removes a small amount of enamel between teeth | Effective for mild crowding; limited total gain; irreversible but conservative when done properly |
| Extraction | Removes teeth to create substantial space | Necessary in severe crowding or where the incisors would otherwise be pushed out of bone; a considered decision, not a default |
Will crowding get worse?
Often, slowly. Lower incisor crowding tends to increase through adult life regardless of treatment history, which is also why retention after orthodontics is indefinite rather than for a fixed period.
Frequently asked questions
Do I need teeth out to fix crowding?
Only if the space deficit is large enough that it cannot be created any other way without pushing the front teeth beyond their supporting bone. Mild and moderate crowding is frequently managed with expansion, enamel reduction, or both. The decision is made from measurements and imaging, not from a policy.
Why did my teeth crowd again after braces?
Because teeth move for life, and because retainers were probably not worn as prescribed. Late lower incisor crowding also occurs in people who never had treatment. Indefinite retention is the only reliable way to hold an orthodontic result.
